Survey of Pediatric Pre-Hospital Care Policies and Equipment
(for EMS agencies)

The purpose of the Ohio EMSC Survey of Pediatric Care Policies and Equipment is to gather general information about your EMS agency in relation to pediatric emergency care. Specifically, the information that the Health Resources and Services Administration (HRSA) requires of its EMSC grantees (i.e., the Ohio EMSC Program) is the following:
1. The percentage of pre-hospital provider agencies in Ohio that have on-line and off-line pediatric medical direction at the scene of an emergency for Basic Life Support (BLS) providers and Advanced Life Support (ALS) providers.
2. The percentage of pre-hospital patient care units in Ohio that have the essential pediatric equipment and supplies, as outlined in the American Academy of Pediatrics (AAP)/American College of Emergency Physicians (ACEP) Joint Guidelines for Basic Life Support (BLS) ambulances and Advanced Life Support (ALS) ambulances.

Participation in this survey is voluntary; however, completing the questionnaire will enable your agency to receive consultation time from the Ohio EMSC Committee with regards to on-line and off-line pediatric medical direction. In addition, your agency will be entered in a drawing to receive a pediatric training mannequin upon completion of this survey.
