EMS Online Test Registration

Welcome to the EMS Online Test Registration.  From here, you will be able to register to take your EMT or EMS Instructor certification test.  You will also be able to register to take an exam for reinstatement if your Ohio certification has lapsed or an exam in lieu of continuing education.  On the online test registration form, you will only be allowed to select to take one examination.

Please note! Effective April 26, 2001, all fire testing will be offered at the program level. We no longer accept registrations for fire certification tests. If you wish to register to take a fire certification test, please contact your training institution. For more information about this change, you can view the letter sent to all fire training program coordinators announcing this change (click here). All fire certification test registrations that were received prior to this announcement will be forwarded to the appropriate training institution.

Important! When you go to the next page to begin the registation process, you will first be asked to enter your Social Security Number and date of birth. If our system does not already have this information, it will not allow you to access the registration. If this occurs, please contact our office to register for an exam.

Before you will be allowed to register for an exam, the online form will ask you to update your certification information. By completing this online form, your certification record will be updated in our office automatically. The information you will be required to verify and update if needed is your address, county of residence, and phone number. You may also add your fax, pager, cell phone, e-mail, and agency affiliation. Please have this information handy to complete the online form.

Once you go to the registration form, you will be asked to select one type of exam you are requesting, the school location, the date that is offered at that location, and the sponsoring school you attended. To the right of the form is a three month calendar you can use to select the best date for you.

After you complete the registration form, click the "Submit Registration" button.  You will then receive one of two messages: your registration has been accepted or there is incomplete information and you will need to again submit your registration.

If your registration is accepted, you will receive a message telling you so and that a confirmation will be sent to you from our office in the mail within three (3) business days after you submit your registration.  If you do not receive your confirmation in the mail, please call our office to confirm your registration.
