EMS Provider Reinstatement

Reinstatement requirements are determined by time elapsed since expiration. A $75.00 application fee (check/money order to “Ohio Treasurer of State”) is required with the reinstatement application.

0-90 Days Since Expiration

  • Option 1
    Provide proof of CE if completed during certification cycle (cannot be completed after expiration date of original certification)
  • Option 2
    Current NREMT certification and completion of Ohio Trauma Triage Course
  • Option 3
    Successful completion of the Ohio Assessment/Reinstatement Test (registration and testing administered by NREMT)

90 Days to 3 Years Since Expiration

  • Option 1
    Current NREMT certification
  • Option 2
    Successful completion of the Ohio Assessment/Reinstatement Test (registration and testing administered by NREMT)

3 Years to 6 Years Since Expiration

  • Option 1
    Current NREMT certification and completion of Ohio Refresher Course (contact an Ohio-accredited training institution)
  • Option 2
    Current NREMT certification and current certification card from another state
  • Option 3
    Successful completion of the Ohio Refresher Course (contact an Ohio-accredited training institution) followed by passing the Ohio Assessment/Reinstatement Test (registration and testing administered by NREMT)

6 Years or More Since Expiration

  • Option 1
    The entire course of training must be repeated and an initial application must be submitted
  • Option 2
    Reciprocity by holding certification from another state and NREMT (click HERE for reciprocity packets)
