An online system has been developed to apply for, renew and manage your medical transportation license. Therefore, we will no longer accept paper applications and forms. All fees must be paid via credit card or Automated Clearing House (ACH).
First-time users to the MTLS Online System must register with Identity Manager.
If you have already registered with Identity Manager, you can access the MTLS Online System by signing in with your email address and password.
The State Board of Emergency Medical, Fire, and Transportation Services is responsible for the licensing of Ohio’s medical transportation services (air medical, ambulance, MoICU and ambulettes), and in accordance with Chapter 119 of the Ohio Revised Code, has adopted rules regulating medical transportation services and vehicles. Rules enforcement, including licensing and inspection processes are carried out by the Ohio Department of Public Safety, Division of Emergency Medical Services.
All medical transportation services (air medical, ambulance, MoICU and ambulettes) operating in the state of Ohio are required to be licensed unless excepted from licensure as set forth in section 4766.09 of the Ohio Revised Code.
